Below are many of our commonly used forms available for you to download. Please mail, fax, or hand deliver completed forms to the Office of the Registrar unless otherwise advised on the form.

Concurrent Enrollment Form		Registration form for registrations at UT Arlington and UT Southwestern Medical Center
Diploma Replacement		Request form to request an additional diploma
Enrollment Certification		Students without a social security number and who have elected to withhold their name and dates of attendance may not use the online service. Please fill out the provided form. Students needing enrollment certifications for auto insurance, scholarships and health insurance may not use the online service. Please fill out the provided form.
FERPA Packet		Request packet includes: Request for Confidentiality, Parents Certification of Dependency, and Third Party Release Request
Name Change Request		This form must be accompanied with proof of change, i.e. valid drivers license, passport, marriage certificate, or court order.
Social Security Number Change Request		This form must be accompanied with a copy of your Social Security card and Comet Card.
Off Campus Transfer Work Approval Form		Mandatory request form for undergraduates taking courses off campus.
Registration Add, Drop, and Withdraw		Please be sure to check the academic calendar for deadlines.
Repeated Course Adjustment		Request form to adjust gpa for courses that have been taken more than once
Residency Questionnaire		Packet to obtain Texas Residency - For registered students ONLY
Texas Tuition Rebate		For Undergraduates Only. Application to receive a $1,000 tuition rebate for qualified undergraduate students. The application is to be filled out in a student's graduating term.
Official Transcript Request		Request form for an Official UTD transcript. Requests can be mailed only to the requestors address on file or to a university address. Please check your address through our student information systems before requesting one sent to your personal address. If you have a hold on your record, your request will not be processed. Once your hold is removed, you will need to resubmit your request.
Undergraduate Change of Major or Addition/Deletion of Degree, Major, or Minor		For Undergraduates Only. Request form for change of major. Eligible students fill out this form with the aid of the new department's advisor.
